Your health information is personal. This policy explains how Staffing NP LLC collects, uses, and safeguards it in keeping with HIPAA and our commitment to your trust.
Last updated: September 17, 2026
To provide your care, Staffing NP LLC collects personal and health information you provide through our intake forms, booking requests, and clinical visits. This may include your name, date of birth, contact details, address, medical history, current medications, allergies, pharmacy preference, and the reason for your visit.
Payment information (such as credit card details) is collected and processed entirely on our secure, third-party payment platform. We never receive, store, or transmit your full card number on our servers.
Your information is used solely to deliver and coordinate your telehealth care — including scheduling visits, conducting consultations, documenting clinical notes, sending prescriptions to your pharmacy, providing test results, and following up on your treatment.
We may also use your contact information to send appointment reminders, visit confirmations, and important administrative notices. You will never receive marketing you did not agree to.
Access to your protected health information (PHI) is strictly limited to Sarah Staff, FNP-BC, and authorized administrative staff who need it to support your care. Every team member is trained on privacy and confidentiality obligations.
We do not sell, rent, or share your health information with any third party for marketing or commercial purposes. Disclosures to external parties (such as a pharmacy or lab) occur only with your consent or as required to fulfill your care.
Your records are stored on a secure, encrypted platform designed for sensitive health data. Access is role-restricted and auditable, meaning only the right people can view your information and every access is logged.
Telehealth visits are conducted over encrypted connections. We follow administrative, technical, and physical safeguards consistent with HIPAA requirements to keep your information confidential and secure.
You have the right to access and request a copy of your health records, request corrections, receive an accounting of certain disclosures, and request restrictions on how your information is used or shared.
You may also request that we communicate with you by alternative means (for example, a different phone number or email) and revoke any consent you have previously given, in writing, at any time.
If you have questions about this Privacy Policy, wish to exercise your rights, or believe your privacy has been compromised, please contact us promptly so we can investigate and respond.
You may reach Sarah Staff, FNP-BC, by phone at 715-538-3258 or by email at staffingnp.llc@gmail.com. We take every privacy concern seriously and will work to resolve it as quickly as possible.
This Privacy Policy may be updated periodically to reflect changes in our practices or legal requirements. Continued use of our services after any change indicates your acknowledgment of the updated policy.
